New Patient Registration

Anyone in England can register with a GP surgery.

You do not need proof of address or immigration status, ID or an NHS number. If you are living in the practice area please contact reception to pick up registration form. 

If you live in our practice area and would like to register with us, please complete one of our registration forms that are available from reception. You will be offered a new patient health check with our Health Care Assistant. This appointment is important as it provides us with the valuable information needed, to enable us to provide the care you need until we receive your full medical records from your previous GP.

Registered patients aged 16 to 78 have a right to an appointment regardless of whether you have been seen or not over the last 3 years. You may be asked to attend a patient health check.

Registered patients aged 75 years and over have a right to an appointment regardless of whether you have been seen or not over the last 1 year. You may be asked to attend for a patient health check.

St Chads Medical Practice aims to give a friendly and professional service to all our patients. Suggestions on how the practice can improve their services are valuable and it is important that we are made aware of any problems as we will then be able to address them.

Every two months we hold a Friends of St Chad’s group. The idea of this meeting is to discuss any issues or ideas you may have with our doctors, nurses and Staff, please leave your contact details if you are interested in joining the group so we can invite you along to the next one.

Please note for the safety of all tenants and visitors to the St Chad’s Centre CCTV is installed within the building.
