How information about you helps us to provide better care

Confidential information from your medical records can be used by the NHS to improve the services offered so we can provide the best possible care for everyone.

This information along with your postcode and NHS number but not your name, are sent to a secure system where it can be linked with other health information.

This allows those planning NHS services or carrying out medical research to use information from different parts of the NHS in a way which does not identify you.

You have a choice.

If you are happy for your information to be used in this way you do not have to do anything.

If you have any concerns or wish to prevent this from happening, please speak to practice staff.

Diabetes

boy_with_inhalerLed by our nursing team, this clinic offers advice and general health check-ups to patients diagnosed with diabetes.

Asthma

Asthma sufferers can be seen in this clinic for advice and support from our nurses who specialise in asthma care.

Family Planning Clinic

Drop in Clinics are held every Tuesday afternoon between 1.00-5.30pm- No appointment is required for Contraceptive advice, Well woman checks, HRT or general advice regarding family planning.

Cervical Screening is between 2.00-5.30pm.

We do not do Coil fittings or Implants - Coil Fittings can be done any other family planning clinic please ask at reception or pick up one of our Family Planning leaflets.

Cervical Screening

If required you can attend one of the family planning drop in sessions held every Monday between 2.00-5.30pm at St Chad’s.

Child health and immunisation

baby_examinationAll new babies are invited for regular check-ups from eight weeks old. You will be contacted by Child Health asking you to contact the surgery to make an appointment. 

The Baby assessments and first immunisations will be booked with the doctor every Thursday ( appointment only).  Childhood immunisations are booked with our practice nurse on a Wednesday Morning 8.30-12.00 (appointment only).

Minor surgery & Joint Injections

Minor operations & Joint Injections can be carried out at the surgery. We hold a minor surgery approximately every four weeks by appointment only. Joint injections can be booked with the doctor at the end of the morning with Dr Gill only.

pregnant51Antenatal

Tuesdays in Sure Start 9.00-12.00.  This clinic is run by the midwives. If you become pregnant, you will need to contact the midwives direct on (0161 633 8417) they are based in the Hollinwood Children’s Centre St Chad’s Centre, Lime Green Parade Limeside. 

The midwife will ask you a few questions and carry out some general health checks. You will be seen regularly throughout your pregnancy either by the local midwives or at the local hospital, or both.

Stopping smoking

Wednesday mornings in Sure Start from 9.00-11.30.  This clinic gives advice and support to patients trying to give up smoking.
